ORDER FORM

Use the form below to order Signed copies or Artwork of your choice.

Note: This will be the email your receipt will be sent to.
Note: This will be what appears on the shipping label. Unless indicated differently in the Questions/Comments section.
Note: At this time, Autographed books are only available to readers in the United States. If you live outside the United States and would like to purchase, please use the contact form to message me.
Two Buyers Notes:
First Note: If you would like a different name for each individual title in your purchase, please indicate which name/inscription belongs with which title in the field.
Second Note: If you would like to purchase more than one copy of a specific title with a different name as the inscription (e.g. gifting a Signed book), you will need to make a separate transaction. A separate transaction also gives you the option of shipping it directly to your gift recipient.

This is only consent for the purpose of this sale.
